Welcome to G4 Physiotherapy & Fitness.
Our aim is to ensure that you receive the most efficient and effective quality of care from our team. We would be grateful if you could read the following carefully and fully accept all prior to the commencement of any therapy or fitness intervention.
Sessions at G4 may involve a physical/visual examination as part of the assessment and treatment process. Therefore, there may be some physical contact from the therapist/instructor.
As part of any therapy assessment & treatment process, you may be asked to undress suitably and perform certain exercises or tests that may make your symptoms initially better or worse.
Our aim is to help to relieve your symptoms but on occasions, treatment may seem to aggravate your condition. This is common as sometimes your symptoms may feel worse initially before they get better. Such aggravation may last a few hours or days. Occasionally, you may experience temporary local bruising or swelling due to various forms of treatment but this will be short-lived. If acupuncture is recommended by the practitioner, please be aware that you can decline the use at any time.
All children below the age of 16 must be accompanied by a parent/guardian. You may be accompanied by a friend or relative if you request during each session.
As part of the service, it is your duty to inform the therapist of all medication that you are taking and of your past/current medical history. Such history includes the presence of any artificial implants, including pace- makers. You must inform the therapist if you are pregnant.
As part of the treatment, rehabilitation and fitness process, you may be asked to perform certain tasks or use certain equipment. You may only carry out those tasks or use the designated equipment as instructed by our team. This applies to both indoor and outdoor rehabilitation areas. If outdoor rehabilitation is carried out, you must understand that some terrains are potentially hazardous and assume and accept that there may be a risk of injury. You do not have to carry out any exercise as instructed by the therapist/instructor if you decide not to. You may ask questions at any time and also ask the therapist/instructor to stop any form of assessment/therapy/fitness whenever you wish.
Please be aware that you may be liable to pay for any missed sessions or any sessions that are cancelled within short notice.
Private Health Insurance - You agree that you are liable to pay in full any excess that is due from your private medical insurance policy if this is confirmed by your medical insurance company.
Self-Paying - You are required to settle each treatment fee on the same day or prior.
By proceeding to book an appointment and having further contact with the clinic, you agree and accept the above terms and conditions.
COVID-19 RISK CONSENT FORM (other G4 services may apply where indicated)
Explanation of Physiotherapy Services during the Coronavirus Crisis with G4 Physiotherapy & Fitness:
Your physiotherapist will complete a Clinical Triage and Risk Assessment, which will help them determine that your condition warrants a face to face consultation, despite the risk of coronavirus infection. Face to face consultations will not be offered for all routine physiotherapy treatment in the current climate, and all precautions to prevent and control the risk of infection must be observed for this consultation to take place.
Your consultation will be with a qualified physiotherapist. It will include detailed discussion about your condition and a physical examination that may require the physiotherapist to touch you or be close to you; it may be necessary to remove articles of clothing to allow examination. If you experience discomfort during the examination, please report this to your physiotherapist, they will not ask you to perform any task which is not appropriate for, or relevant to, your injury/condition.
The physiotherapist will be wearing personal protective garments and equipment during the consultation and will maintain a minimum 2m distance from you, unless closer contact or observation is required for short periods of time. Please ensure that you maintain this distance yourself during the consultation. You should provide yourself with PPE (face covering minimum) during the consultation.
Following your assessment, a course of treatment may be recommended, which may be provided in clinic (face-to-face) or virtually by telephone or video calling. Your physiotherapist will discuss and agree treatment plans with you; these will always include self-management strategies, and exercises for you to do at home on a regular basis.
Because of the risk of coronavirus infection, it is important that we make you aware that there is an increased risk of coronavirus infection if you choose to attend a face to face consultation, despite all precautions being taken.
