Client Information: On booking your appointment you will receive a Booking Confirmation. Please ensure you read all of this information carefully. Please ensure you read the information below. Location & Directions: 121 Theobald St, Borehamwood, WD6 4PT Direction and parking details can be found at https://www.borehamwoodphysio.co.uk/contact.html (PLEASE COPY AND PASTE THIS LINK) Borehamwood Physio is located in a Private room and is based at the NHS GP Manor View Practice at Theobald (formally Theobald Medical Centre, which is still the name on Google Maps). Borehamwood Physio can be found upstairs within the NHS Medical Centre. Please do not sit downstairs. If you are unable to use stairs, please contact the clinic as another room can be arranged for your sessions. On arrival please be aware that Borehamwood Physio has no link to the GP practice. The receptionists are for the NHS GP Surgery. If you are stopped and asked, explain you are seeing the PRIVATE PHYSIOTHERAPIST. Please DO NOT wait downstairs. Please enter the GP practice and walk past the reception desk, which will be on your left. Just before the end of the desk there is a door on your right and a small sign that says Borehamwood Physio. Please enter the door and walk up the stairs. At the top of the stairs enter the first door on your right for the small waiting room. It says waiting room above the door. In the waiting room you will see a closed door with the sign Borehamwood Physio. Please take a seat and you will be called for your appointment. Disabled Access: The private treatment room is located upstairs, within the NHS practice. It is not part of the NHS GP Practice. Please be aware that you can request a DOWNSTAIRS ROOM if you are unable to manage stairs. Please ensure 24 hour notice so that a downstairs treatment room can be requested. Length of appointment: Initial assessment standard: 60 minutes Initial assessment COMPLEX referrals: 90 minutes Follow-up: 30 minutes Extended follow-up appointments are also offered if needed: 45 minutes and 60 minutes. Insurance companies: Form completion: IPRS, including Avia and Vitality and BUPA Please be aware that we are required to complete assessment forms throughout your treatment or when requesting further sessions. Time is allocated during your appointment to complete these. Payment: At Borehamwood Physio we believe that we offer Physiotherapy at a reasonable fee, which also takes into account the practitioners years of experience as well as business expenses. Sadly some insurance companies do not cover the full cost of the Physiotherapy appointment, and only pay the clinic a reduced fee. In these instances you will be asked to pay the remainder of the fee at your appointment. The clinic will still bill the insurance company for the other part of the payment. These companies are currently IPRS, Vitality and AVIVA. Cancellation Policy: We require 24 hours’ notice for a cancellation. Late cancellations: The full payment for the appointment will be charged for under 24-hour notice and will be expected to paid before booking your next appointment. If repeated appointments are cancelled or you fail to attend then this and may result in treatment being withdrawn from the clinic. Payments must be made within 14 days. Failed attendance: A payment of 100% of the appointment fee will be charged and must be paid before further appointments are made. Repeat failures may result in treatment being withdrawn from the clinic. Payments must be made within 14 days. Insurance claims: In accordance with your insurer policy, you will either by liable for the cost or have a treatment session detected for late cancellations and failed attendance. Payments: Pre-Payment Deposit: In some instances you may have to pay for the session in advance such as part payment deposit. This will be sent via a payment link or through the online booking system. If you need to cancel your appointment and have given over 24hrs notice this payment will be refunded. In Clinic: Self-funding Clients: Payment will be made in clinic by card only. Virtual appointments are made through an i-zettle link sent to you to complete payment or by BACS details sent in specific circumstances. Private Medical Insurance Claims: At Borehamwood Physio we believe that we offer Physiotherapy at a reasonable fee, which also takes into account the practitioners years of experience as well as business expenses. We can bill some insurers directly for your treatment. However, you remain responsible for the cost of your treatment and the charge will automatically revert to you if your insurer does not make payment in full within 60 days. Any excess must be paid in full when required, within 14 days. This can be made by BACS or a payment link. Sadly some insurance companies do not cover the full cost of the Physiotherapy appointment, and only pay the clinic a reduced fee. In these instances you will be asked to pay the remainder of the fee at your appointment. The clinic will still bill the insurance company for the other part of the payment. These companies are currently IPRS, Vitality and AVIVA. Consent to treatment: Consent will be documented in your notes at your first appointment. Please be aware that during your appointment you will undergo a clinical assessment as described above. You have the right to refuse treatment at any time and ask the physiotherapist to stop, withdrawing your consent. Client responsibilities: If a letter is written for a GP, consultant or other third party in relation to your treatment for example for further investigation, onward referral or imaging, you will be responsible for arranging your appointment with them. Borehamwood Physio will ensure that the letter is written and sent. You will be bound by the terms and conditions of that third party and we undertake no liability with respect to the third party. Under 16 years of age: If you are under 16 years of age you must be accompanied by a parent or guardian who will always be present . During your appointment: At your initial appointment time will allowed to discuss in-depth your presenting complaint, your goals and any other relevant history. You will then undergo an examination and treatment, Pilates session or Run assessment depending on what you appointment is for. What to wear: In order to complete a thorough assessment you may be asked to remove articles of clothing. Shorts or trousers that can be rolled up are recommended for all leg and back complaints. If you prefer you can wear a vest top with a low back and thin straps for assessment of the back, neck and arm. For Pilates please wear comfortable clothing for the session. Please ensure you have an exercise mat and pillows. For the Run Assessment please wear shorts, a vest or t-shirt and running trainers. What to bring: Any information related to your referral including insurance details, medical information and previous treatment notes. Please wear appropriate clothing and footwear.
